What Is TMD?

TMD -- temporomandibular disorder -- is the current term for what most people call TMJ. It is actually a collection of related conditions affecting the jaw joint (the temporomandibular joint), the chewing muscles, and the surrounding structures.

The jaw joint itself is one of the most complex joints in your body. It can hinge open and closed, slide forward and backward, and shift side to side. It contains a disc similar to the ones in your spine, and it is surrounded by powerful muscles that generate enormous bite forces.

About 90 to 95% of TMD cases are muscular in nature -- meaning there is no structural damage to the joint itself. Instead, the muscles around the jaw are in spasm, overworked, or dysfunctional, often due to clenching, grinding (bruxism), or stress.

Here is something most people do not know: your jaw and your neck share embryological origins. During fetal development, the structures that form the jaw, the inner ear, and parts of the neck all come from the same tissue. Three Categories of Causes

TMD typically involves three types of contributing factors:

  • Predisposing factors -- your individual structural, neurological, hormonal, and metabolic makeup. Some people are simply more prone to jaw dysfunction.
  • Precipitating factors -- things that trigger the problem. This includes direct trauma to the jaw, repetitive habits like nail biting or gum chewing, teeth clenching or grinding, and stress that exceeds your personal threshold.
  • Perpetuating factors -- things that keep the problem going. Systemic disease, cervical spine dysfunction, ongoing stress, anxiety, and depression all fall into this category.

TMD affects women more often than men, particularly women over age 55. The National Institutes of Health has noted that most TMD symptoms are self-limiting but can recur and fluctuate over time.
